Description

*Hybrid role, candidate must reside in the State of Nevada in one of the following counties; Carson County, Douglas County, Lyon County, Storey County or surrounding areas.*

  • Conducts outreach for services engagement for members identified by the Nevada judicial system and supports members SDoH needs
  • Develops and maintains relationships with external stakeholders such as judges, court, housing providers, community-based organizations, and provider personnel.
  • Communicates court related follow-up/requirements to the interdisciplinary team and collaborates with the interdisciplinary team to achieve optimal resource outcomes.
  • Assists in the deployment of the state contract deliverables.
  • Provides support to care and case managers regarding the coordination of care plans for members by utilizing social service expertise to evaluate the members need for alternative services and third-party intervention.
  • Performs in-reach activities and care coordination at courts, treatment centers, housing & shelter programs, and judiciary institutions.
  • Knowledge and ability to navigate social, economic, housing, judicial barriers and current issues impacting the housing insecure and judicial population.
  • Ability to coordinate the delivery of public assistance programs and resources to customers and their family.
  • Performs other duties as assigned
  • Complies with all policies and standards

Education/Experience:

Bachelor's degree in Criminal Justice, Counseling, Social Work, Public Health , Sociology or related field required. Master's degree preferred. 2+ years of related experience in Health Insurance, Criminal Justice, Healthcare Navigation, Supportive Housing Services, or Reentry Services.

License/Certification:

Citizenship Status Required. Must have a valid Driver's license due to required travel.

License/Certification:

Citizenship Status Required, must be able to Pass Las Vegas Metro Police Department Background Check, Nevada Department of Corrections Background Check for facility access. *This is a condition of ongoing employment to fulfill role. Valid & current Nevada Driver's License required due to required travel.

Pay Range: $55,100.00 - $99,000.00 per year

Centene offers a comprehensive benefits package including: competitive pay, health insurance, 401K and stock purchase plans, tuition reimbursement, paid time off plus holidays, and a flexible approach to work with remote, hybrid, field or office work schedules. Actual pay will be adjusted based on an individual's skills, experience, education, and other job-related factors permitted by law, including full-time or part-time status. Total compensation may also include additional forms of incentives. Benefits may be subject to program eligibility.
