We collaborate with our facility partners to deliver evidence-based mental health care services, including psychiatric assessments, medication management, psychotherapy, crisis intervention, hospital placements and treatment, educational training, and quality regulatory documentation. Position Overview: We are currently seeking a Nurse Practitioner to join our team and provide psychiatric care services at our partner facilities. Responsibilities: Provide on-site psychiatric assessments and medication management to patients in skilled nursing, assisted living, and other long-term care facilities. Consult, educate, and coordinate care with facility staff, including nursing home administrator, director of nursing, and director of social services. Adhere to established regulations, policies, and procedures while exercising initiative and independent judgment.
